| ميزات القسطرة عن طريق شرايين الرسغ Features of the catheter through the arteries of the wrist
القسطرة عن طريق اليد (شرايين الرسغ) هي الطريقة الأحدث في عمل القسطرة والتي يسعى كل مركز قلب متقدم في العالم لأن تكون الطريقة الروتينية في جميع حالات قسطرة شرايين القلب، وفي اميركا مثلاً يتمنون ان تكون هي الطريقة المعتمدة في عام 2030 في 40% من المراكز في اميركا. والسبب في قلة استخدام هذه الطريقة حالياً او ندرته هو انها تحتاج لفترة تدريب اطول فهي صعبة على الطبيب ولكنها سهلة على المريض والطاقم الطبي المساعد، فهي اسهل على المريض لانها لا تلزمه الاستلقاء الدائم على الظهر طوال عمل القسطرة، كما ان المريض ينزل مباشرة عن طاولة القسطرة ماشياً ولا تلزمه بعدم صعود الدرج لمدة اسبوعين بل يستطيع المريض عمل ما يشاء في اليوم التالي، كما انه لا يلزم تنويم المريض اذ يكفي بقاؤه في اي استراحة لمدة ساعتين ثم يغادر المستشفى.
والأهم من هذا هو تجنب المشكلات المحتملة من القسطرة عن طريق شريان الفخذ مثل التورم الدموي او تسرب الدم حول البطن او النزيف او تفسخ الشريان وحدوث الوصلات بين الشرايين والاوردة. وهذه المشكلات قد تؤدي الى الوفاة احياناً.
وبالمقارنة مع القسطرة عن طريق شريان الرسغ فان المشكلات لا تكاد تُذكر اذ اسوأها ان ينقفل الشريان وفي هذه الحالة غالباً لا يشعر المريض بأي خلل لأن الرسغ فيه شريانان او ثلاثة تؤدي الغرض تماماً، كما أن هذا الانقفال رجعي وفي اغلب الاحيان يعود وينفتح وحده. عدا عن هذه الميزات فان المرضى كبار السن اولئك الذين يعانون من آلام الفقرات والظهر والذين لا يستطيعون التبول وهم مستلقون على ظهورهم فان هذه الطريقة تمنحهم الحرية التامة للحركة وتجنبهم هذه المعاناة.
والأكثر اهمية هو امكانية عمل قسطرة شرايين القلب عند المرضى الذين يأخذون ادوية مسيلة للدم، ففي العادة يطلب من هؤلاء المرضى ان يوقفوا الدواء المسيل للدم لغاية عمل القسطرة عن طريق شرايين الفخذ، بينما يعمل القسطرة بالطريقة الحديثة عن طريق شريان الرسغ، فلا يلزم ايقاف اي علاج مسيل للدم، بل حتى عند هؤلاء المرضى المعرضين للنزيف الشديد مثل الذين أخذوا دواء مذيب الجلطات او يأخذون دواء الوافرين المسيل الشديد، حتى هؤلاء تعمل لهم القسطرة عن طريق شريان الرسغ دونما مخاطرة للنزف.
اضف لهذه الميزات ان القسطرة بالطريقة الحديثة هذه تقلل من حدوث جلطات الدماغ التي قد تحصل خلال القسطرة بالطريقة التقليدية القديمة، ومن الميزات الاخرى ان هذه القسطرة الحديثة أقل تكلفة اذ لا يحتاج المريض للنوم في المستشفى ويتم عمل القسطرة التشخيصية باستخدام انبوبة قسطرة واحدة فقط في معظم الأحيان.
ومن الأمور اللطيفة في هذا الاجراء الابتعاد عن منطقة الحشمة اذ ان الشريان الفخذي يمر قريباً جداً منها، فهي منطقة حساسة للألم وحساسة من الناحية النفسية وخاصة عند النساء، وبعمل القسطرة عن طريق شريان الرسغ يشعر المريض او المريضة براحة اكبر.
وبسبب هذه الفوائد الكثيرة، فان جميع مراكز القلب في العالم تشجع كادرها للتحول من عمل القسطرة عن طريق شريان الفخذ الى القسطرة عن طريق شريان الرسغ بل وتحاول جاهدة لتدريب الجيل الصاعد من اختصاصيي القلب والقسطرة على هذه الطريقة الحديثة.
